Page two of this document is the required Medication use form. This form must be filled out for each medication that you wish for your child to receive while at school. A new medication form must be filled out each school year.
Self-carry medications
Under special circumstances, Aubrey ISD will permit a child to carry, and self-administer certain medications. The Board of Education policy permits a responsible, trained student to carry and/or self-administer medication for asthma (wheezing), severe allergic (anaphylactic) reaction, or diabetes on his/her person for immediate use in a life-threatening situation with written order of physician, parent request, school nurse and principal approvals.
Please click the link below for the self-carry forms. It is a 2 page document and must be completed by all responsible parties prior to the child bringing the medication for self-administration. A new self-carry form must be filled out each school year.
NOTE: A NEW FORM MUST BE FILLED OUT EACH SCHOOL YEAR
